| Proteinuria (3+) and mildly elevated creatinine (0.8) found in June 2004 when evaluated by PMD for fatigue, lethargy. Patient was noted at that time to have had a 13 lb weight loss since November 2003. On recheck 6/21/04 urine protein was negative; creatinine was 0.7, albumin 2.7. Patient remained fatigued for several more weeks but denies any fever, edema or other symptoms. She states that her energy level is back to normal currently. She has continued to lose weight despite a good appetite. She eats a hot breakfast and lunch daily at school, a snack after school and dinner. She drinks milk with lunch, 2 glasses a day of water and “lots” of soda. Heavy proteinuria and mild nephrotic syndrome was noted by Sep 04. Percutaneous renal biopsy 9/24/04 revealed focal segmental and global glomerulosclerosis with minimal interstitial changes |
